Summary:
The LVN provides routine and emergency assessments and nursing care to ensure maintenance of optimal health status of each individual. Maintains accurate and complete documentation of all health services provided to individual. Participates in the development, implementation, and monitoring of each individual’s service plan.
Essential Functions and Responsibilities:
  • Performs health care procedures and monitors individuals’ health conditions.
  • Provides for administering of medication.
  • Assesses and monitors the individual’s use of medication.
  • Assesses and monitors the health data and information.
  • Assists individuals in securing emergency medical services.
  • Assists individuals in obtaining appropriate medical services.
  • Performs health care procedure’s ordered/prescribed by a physician/medical practitioner and required by standards of professional practice or law to be performed by licensed nursing personnel.
  • Immediately reports to supervisor any significant changes in an individual’s health status including, but not limited to, acute illness, adverse reactions to medication, sudden and/or unexplained changes in vital signs, increase seizure activity, etc.
  • Immediately reports to supervisor any problems related to individual safety, staff problems, and/or facility problems.
  • Provides training for unlicensed Team Members including, but not limited to, training in vital signs and medication reasons, times, and side-effects.
  • Assesses Unlicensed Team Member, (DSP, UAP, staff) proficiency at least quarterly.
  • Trains individuals and their families on chronic medical problems.
  • Documents all nursing interventions at the time they are completed in SDLs or Nurses Notes, and maintain a 50% billing rate in Waiver Services.
  • Monitors individual records to ensure compliance with physicians’ orders and appropriateness of nursing interventions.
  • Ensures that all medical consultations and laboratory reports are documented in the individual’s records in a timely manner.
  • Ensures nursing notes contain subjective, observation, and assessment documentation, plan of correction and treatment documentation and resolution documentation of all individual physical injuries.
  • Participates as assigned in annual and interim planning conferences for individuals served.
  • Recommends individual-training objectives in the areas of health maintenance, oral hygiene and self-administration of medications based on identified individual needs.
  • Assists case manager in development of health care training objectives as requested and ensures that staff is knowledgeable in program implementation.
  • Monitors medication storage area to ensure that all medications are stored and labeled properly and that outdated and/or discontinued drugs are removed and disposed of according to standards of practice.
  • Provides in-service training in health-related areas to direct care staff as needed.
  • Immediately responds to emergency calls relative to heath and safety of individual’s.
  • Serves as member of agency-wide committees as assigned.
  • Completes special assignments and submits written reports within designated time frames.
Qualifications:
  • Knowledge of accepted standards of nursing practice and ability to apply knowledge in daily work activities.
  • Ability to communicate with community health care professionals and interpret their recommendations to other staff.
  • Ability to assess individual’s health status and to provide appropriate interventions
  • Ability to organize workload to ensure continuity of care of individuals served.
Education and/or Experience:
  • Graduation from an accredited nursing program and possession of valid LVN license from the Texas Board of Nurse Examiners
  • One year nursing experience
