Delivering great healthcare begins with you! That’s because our Case Coordinators are an integral part of the vybe urgent care team!. The Case Coordinator is responsible for coordinating and monitoring a patient’s case throughout the patient experience. This will include but is not limited to scheduling of appointments, updating and maintaining appropriate patient care documentation, coordinating with insurance providers & employers, and being the liaison between vybe and other healthcare providers.

ESSENTIAL DUTIES AND RESPONSIBILITIES

Primary duties include the following, with oversight and direction provided by the VP of Finance, as needed:

  1. Responsible for worker’s comp claim coordination:
  1. Initiate contact with patient/patient's employer to obtain necessary information related to WC claim
  2. Collect and maintain accurate patient records and update information, as needed (i.e. Claim number, WC insurance carrier)
  3. Ensure EMR records are properly updated and maintained
  4. Serve as liaison between patient's employer and third party RCM provider to ensure accurate patient billing and timely collections
  5. Handle inquiries and provide information to patients, patient's employer and/or WC insurance provider regarding WC services
  1. Responsible for case coordinator with the Jefferson and HP Programs:
  1. Initiate contact with discharged patients participating in specific payor programs to coordinate post-discharge care; understand the care needed and direct him/her to the applicable centers.
  2. Establish and maintain accurate and complete patient records which will be shared with the payors for the applicable programs.
  3. Ensure EMR records are properly updated and maintained
  4. Serve as liaison between payor program, patient and vybe providers to ensure proper care is delivered
  5. Proactively communicate with patients post vybe visit to ensure adequate care was provided
  1. Other duties as assigned or as required for effective and safe center operation

POSITION REQUIREMENTS

  1. High School Diploma or equivalent required.
  2. 3-5 years experience in healthcare / medical support related positions required.
  3. Familiarity with insurance terminology, worker's compensation, and electronic medical records highly preferred.
  4. Excellent organizational and interpersonal communication skills and the ability to build and maintain effective working relationships across various constituencies.
  5. Must convey a professional demeanor and be able to act calmly and effectively in a busy or stressful situation.
  6. This position requires access to confidential and sensitive information, requiring ongoing discretion and secure information management.

WORKING CONDITIONS

  1. This position is based in one or more urgent care centers.
  2. Potential exposure to moderate noise and infectious diseases and blood-borne pathogens.
  3. Requires travel to other centers and meeting locations throughout Philadelphia.
  4. Physical requirements include bending, reaching, standing, walking, squatting and sitting, pushing and pulling throughout a regular work shift. Requires manual dexterity and ability to occasionally lift 50 lbs.
